Background

Doctori is a clinical management platform built for the way small and mid-sized clinics actually operate — several practices, shared infrastructure, and no IT department between them.

The brief was not a booking form. It was the whole operating layer: patient records, scheduling, billing, and the messaging channels patients already use, with each clinic strictly isolated from every other one on the same system.

The challenge

Multi-tenancy in healthcare is not a routing problem, it is a data-isolation problem. One clinic must never see another clinic’s patients, and that guarantee has to hold at the database layer rather than depend on application code being correct everywhere.

The second difficulty was reach. Clinic patients in this market do not check email or log into portals — they message on WhatsApp and Instagram. A platform they have to be taught to use is a platform they will not use.

Our approach

We built isolation in at the data layer, so a tenant boundary is enforced by the system rather than remembered by a developer. That decision costs more up front and is the reason the platform can add a clinic without a security review each time.

For reach, we inverted the usual model: instead of asking patients to come to a portal, the platform goes to the channels they already have open. Appointment confirmations, reminders and follow-ups arrive on WhatsApp or Instagram, and replies come back into the same clinical timeline the practice works from.

The AI clinical assistant was scoped deliberately narrowly — it supports the practitioner with drafting and retrieval inside the record, and does not make clinical determinations. In healthcare, the boundary of what the AI is allowed to do is a product decision, not a technical one.

The outcome

Doctori runs live as a multi-tenant platform: a new clinic is onboarded onto shared infrastructure without a bespoke deployment, and without touching another tenant’s data.

For SmartTech it is the clearest proof that the studio ships production software with real billing, real third-party integrations, and real isolation requirements — not marketing sites with a login bolted on.
